Point-of-Care Clinical Decision Support
POC Decision
Point-of-Care Decision tools designed to support time-sensitive clinical reasoning at the bedside—especially during the first critical minutes of emergency and ICU care. Tools 01 and 02 open in Claude.ai and may require a free sign-in. Tool 03 runs directly on this website without a Claude.ai account.
Legal & Clinical Disclaimer: These tools are provided for educational and clinical decision-support purposes for qualified health professionals. They do not establish a diagnosis, prescribe treatment, replace institutional policies or specialist consultation, or substitute for independent professional judgment. Clinical decisions remain the responsibility of the treating clinician and should be based on the individual patient, current evidence, local protocols, and available resources.
POC Decision Tool 01
Empirical Antibiotic Therapy
First-hour decision support for the ED and ICU.
This interactive tool is designed to assist clinicians with the initial choice of empirical antimicrobial therapy when a patient presents with a suspected serious infection and definitive microbiology is not yet available.
Its purpose is to make the first-hour reasoning process faster and more structured while keeping the final treatment decision with the treating clinician.
Using this tool may require signing in to Claude.ai. Sign-up is free.
Designed for
The first hour
Emergency department · ICU · critically ill patients · suspected serious infection · microbiology pending
Think beyond the drug:
source · severity · host factors · prior cultures · resistance risk · organ function · source control
POC Decision Tool 02
Pulmonary Embolism Console
Point-of-care clinical decision support for suspected or confirmed pulmonary embolism.
This interactive console is designed to support structured bedside reasoning around pulmonary embolism evaluation and management. It is intended to complement clinical assessment, institutional pathways, and current evidence—not replace them.
Using this tool may require signing in to Claude.ai. Sign-up is free.
Designed for
Pulmonary embolism decisions
Emergency department · ICU · inpatient medicine · suspected PE · confirmed PE · risk stratification · treatment planning
Use alongside:
clinical probability · hemodynamics · imaging · biomarkers · bleeding risk · contraindications · institutional protocols
POC Decision Tool 03
PEEP Optimization in Obese Patients with ARDS
Personalize PEEP with two practical ICU pathways.
A bedside clinical decision-support tool for critically ill patients with obesity and ARDS. Choose A. Esophageal manometry when validated esophageal-pressure monitoring is available, or B. Compliance-guided PEEP titration when it is not.
Includes stepwise safety checks, transpulmonary and driving-pressure calculators, physiological interpretation and linked references. No Claude.ai sign-in required.
Designed for
Precision PEEP
Critical care · obese patients with ARDS · lung-protective ventilation · transpulmonary pressure · respiratory-system compliance
Think beyond a single pressure:
PBW-based VT · plateau · driving pressure · recruitment · oxygenation · hemodynamics · right-ventricular tolerance
How to use POC Decision
Fast enough for the bedside. Structured enough to make the reasoning visible.
1 · Define the problem
Clinical context first
Start with the patient’s syndrome, severity, likely source, and the immediate decision that cannot wait.
2 · Structure the choice
Make assumptions explicit
The tool organizes key bedside variables so the clinician can see why a recommendation is being considered.
3 · Apply judgment
Decision support—not autopilot
Reconcile the output with allergies, organ function, prior microbiology, local resistance patterns, source control, and the complete clinical picture.
Clinical guardrails
Empirical antimicrobial therapy is a high-stakes clinical decision. The POC tool should be used as an adjunct to—not a replacement for—bedside assessment, institutional protocols, infectious diseases consultation when appropriate, pharmacy input, and the local antibiogram.
- Confirm allergy history and the nature of prior reactions.
- Account for renal and hepatic function, weight, pregnancy status, drug interactions, and prior antimicrobial exposure.
- Review prior cultures, colonization, resistant organisms, recent hospitalization, and health-care exposure.
- Obtain appropriate cultures promptly when feasible without causing unsafe delay in treatment.
- Reassess therapy as microbiology, imaging, source control information, and the clinical trajectory become available.
- De-escalate, narrow, or stop therapy when the evolving evidence supports doing so.
More POC Decision tools are coming
This page is designed as a growing bedside decision-support library. Future tools can use the same framework: define the time-sensitive clinical problem, make the decision variables explicit, and support—not replace—clinician judgment.
Educational and clinical decision-support disclaimer: POC Decision tools are intended for qualified health professionals. They do not establish a diagnosis, prescribe treatment, replace institutional policies, or substitute for independent clinician judgment. Recommendations must be interpreted in the context of the individual patient, local microbiology and resistance patterns, medication availability, and current evidence.
