Premature closure is often a failure to reopen the case when new data no longer fit

Lede: The dangerous moment is not always when the first diagnosis is made—it is when contradictory evidence appears and the diagnosis is not reconsidered.

Why it matters

Premature closure occurs when diagnostic reasoning stops too early. Once a plausible diagnosis is accepted, subsequent findings may be selectively interpreted to preserve it. A practical countermeasure is to deliberately identify discordant data, ask what alternative diagnosis better explains them, and reassess the working model when the patient’s course deviates from expectation.

The pearl

When the clinical course stops behaving the way your diagnosis predicts, reopen the differential before adding more exceptions to the original story.
