A good problem representation is often more valuable than a long differential
Lede: The quality of the diagnosis often depends on how the case is mentally compressed before the differential is generated.
Why it matters
Problem representation is a concise, evolving synthesis of the diagnostically important features of a case. Effective representations use semantic qualifiers and emphasize discriminating positives and negatives rather than reproducing every data point. This helps activate relevant illness scripts and makes competing diagnoses easier to compare.
The pearl
Before expanding the differential, improve the one-liner. If the problem representation is vague, the differential will usually be vague too.
